INDIVIDUAL REGISTRATION
In consideration of being allowed to participate in any way in ProtecHockey Spring Elite 3on3 League and
related events and activities, I , the undersigned, do hereby:
Agree that prior to participating, I will inspect the facilities and equipment to be used, and if I believe anything
to be unsafe, I will immediately advise, in writing, the director of such conditions and refuse to participate.
Acknowledge and fully understand that each participant is voluntarily engaging in activities that involve risk of
injury (even catastrophic injury) which might result not only from their own actions, in-actions, or negligence
but the actions, inactions, or negligence of others, the rules of play, or the conditions, premises or any of the
equipment used, and further, that there may be risks not know to either party or not reasonably foreseeable
at this time. Assume all the foregoing risks as a condition of participation and accept personal responsibility
for the damages following any such injury and do hereby hold harmless ProtecHockey Ponds and any one
affiliated in any way with ProtecHockey Ponds. Acknowledge that it is the participant’s responsibility to be
properly insured and/or pay all medical costs in the event of an injury. Intending to be legally bound, release,
waive, unconditionally discharge and consent not to sue or otherwise pursue any claims against
ProtecHockey Ponds, its owners, directors, administrators, instructors, agents, coaches, trainers, volunteers,
and any other person or entity connected with the league, its sponsors, and/or Protechockey Ponds or any
other facilities ProtecHockey Ponds is involved with for any claims, demands, losses or damages on account
of injury, including death and permanent disability, or damage to property caused or alleged to be caused in
whole or in part by the negligence, other actions or inactions, or otherwise in connection with association
and/or arising out of the participation in any or all of the ProtecHockey Ponds official or unofficial activities,
learn to skate, events, or competitions.
Consent to accept responsibility for final decisions regarding continued participation if suffering from injuries.
In addition, the undersigned certifies that the participant is covered by the submitted health insurance plan
during participation in ProtecHockey Ponds Spring Elite 3on3 League and accepts the potential costs for
treatment that may or may not be covered by the stated insurance plan. Hereby consent to allow my picture
and/or likeness to appear in any official documentary or promotional materials, or in any publication
developed for ProtecHockey Ponds in any manner incidental to participation in ProtecHockey Ponds without
my prior consent or compensation to me. Agree to conduct myself in a non-violent, non-disruptive manner at
all the times, and, further, understand that anyone not conducting themselves as such may be immediately
dismissed from the league and premises and forfeits all fees paid.
CANCELLATION AND CREDIT POLICY
PROTECHOCKEY PONDS policy is that there shall be NO REFUNDS in the event of participant withdrawal or
cancellation for whatever reason, including anticipated or unanticipated injury or illness. THERE WILL BE NO
MAKE-UPS OR CREDITS FOR MISSED GAMES. ANY CUSTOMER DROPPING OUT OF THE PROGRAM
WILL STILL BE RESPONSIBLE FOR PAYING THE FEE FOR THE ENTIRE SEASON.
